ALL DAY CARE CENTER

REGISTRATION PROCESS
Welcome to All Day Care Center. Below yow will find the procedures that will be required as you enroll in our day center. First step is a tour of the center followed by a review of the activities calendar and our menus. During this meeting we will conduct a short memory screening and a recreation assessment. Next, we will arrange for a complimentary visit to determine if the program is appropriate for you or your loved one. If this visit is favorable, we will need a completed physician certificate (form provided by the center) that identifies any health conditions, allergies, prescribed medications and proof that you are free of tuberculosis or other communicable diseases. You and your family member will meet with our nurse to complete the forms for your chart.
At this time we will need the following:
1. Social security number
2. Date of birth
3. Health insurance information
4. Emergency contact information
5. Physician information
6. Signed contract for days of attendance
